How to Choose a Medical Billing Company: 9 Questions That Reveal the Truth

The right medical billing company protects your revenue; the wrong one quietly drains it. Here are nine questions that separate a real partner from a claim-dropping vendor.

By Novusmedix

Every medical billing company will tell you they'll increase your collections. The pitch sounds the same across the board. So how do you tell a genuine revenue partner from a vendor that just forwards claims and takes a cut?

You ask better questions. The nine below are the ones that actually expose how a company works — because a strong billing company answers them easily, and a weak one gets vague fast.

1. What is your clean claim rate on first submission?

This is the single most revealing number. The clean claim rate is the percentage of claims accepted by the payer the first time, with no rework. A strong operation runs high here. If a company can't give you a number — or dodges the question — that tells you they aren't measuring the thing that matters most.

2. How do you handle denials — specifically?

Listen for the difference between "we resubmit denied claims" and "we work each denial to its root cause, appeal when it's wrong, and fix the upstream process so it stops recurring." The first is busywork. The second is what recovers real money and prevents the same denial next month.

3. Who works my accounts receivable, and how often?

Aging claims are where revenue quietly dies. Ask how often they touch your A/R, how they prioritize old claims, and what your average days-in-A/R would look like. A company that lets claims sit until they're too old to appeal is costing you more than its fee.

4. Do you do coding, or only billing?

Billing and coding are deeply connected. When the same team handles both, coding errors get caught before they become denials. If a company only submits claims and leaves coding entirely to you, you're carrying the risk that causes most rejections.

5. What reporting will I get, and how often?

You should receive clear, regular reports on clean claim rate, denial rate, days in A/R, and collection rate — in language you can read. If the answer is "we'll send you something monthly" with no specifics, you'll be flying blind on your own revenue.

6. How do you verify eligibility and handle prior authorizations?

Most preventable denials trace back to eligibility not being checked or an authorization not being obtained. A serious company treats these as front-end steps built into their workflow, not an afterthought.

7. Who is my point of contact, and how fast do you respond?

When a payment problem hits, you need a human who knows your practice — not a ticket queue. Ask who you'll actually talk to and what their response time is. Communication quality predicts the whole relationship.

8. How is my patient and practice data protected?

You're handing over protected health information. Ask directly how they safeguard data and stay compliant. A confident, specific answer here is a baseline requirement, not a bonus.

9. How do you charge, and what's included at that rate?

Most companies charge a percentage of collections. The trap is comparing rates without comparing scope. A lower rate that only covers claim submission often collects less net revenue than a slightly higher rate that includes denial work and A/R follow-up. Always ask what the number actually buys.

The pattern to watch for

Notice what these questions have in common: they all push past the sales pitch toward how the company actually operates day to day. A real billing partner welcomes them — the answers are their strength. A weak vendor deflects, generalizes, or changes the subject to price.

Your revenue cycle is too important to hand to whoever quotes the lowest percentage. Choose the company that can show you, in specifics, how they protect every dollar.

Frequently asked questions

Not necessarily. Large companies bring scale, but small practices can get lost in them. What matters more than size is the clean claim rate, how denials and A/R are worked, and whether you get a responsive point of contact who knows your practice.
